seeking a compassionate and mission-driven Pediatric Nurse
Practitioner (PNP) or Family Nurse Practitioner (FNP) with a
pediatric focus to join our Federally Qualified Health Center
(FQHC) care team. This role provides comprehensive, team-based
primary care to infants, children, and adolescents in a community
health setting that serves patients regardless of insurance status
or ability to pay. The ideal candidate is deeply committed to
health equity, trauma-informed care, and whole-child wellness , and
is passionate about supporting families through preventive care,
early intervention, and chronic disease management. Clinical Care
Provide comprehensive pediatric primary care including well-child
visits, acute care, chronic disease management, and developmental
screenings for patients from birth through adolescence. Perform
thorough physical exams and age-appropriate assessments, including
growth, developmental, behavioral, and psychosocial evaluations.
Diagnose and treat common pediatric illnesses, injuries, and
chronic conditions such as asthma, ADHD, obesity, diabetes, and
behavioral health concerns. Administer and manage immunizations in
accordance with CDC and VFC (Vaccines for Children) guidelines.
Prescribe medications, order labs and imaging, and interpret
diagnostic results to guide treatment. Preventive & Family-Centered
Care Provide anticipatory guidance and health education to parents,
caregivers, and patients related to nutrition, safety, sleep,
mental health, and developmental milestones. Screen for social
drivers of health (food insecurity, housing instability,
transportation barriers, etc.) and coordinate referrals to care
teams and community resources. Support families with
trauma-informed, culturally responsive communication in a diverse
community health setting. Care Coordination & Team-Based Practice
Collaborate with pediatricians, behavioral health providers,
medical assistants, care coordinators, and school-based teams to
ensure integrated, whole-child care. Participate in care
conferences and clinical huddles to support patients with complex
medical, developmental, or social needs. Coordinate referrals to
pediatric specialists, therapy services, and community agencies.
Maintain accurate and timely EHR documentation in compliance with
FQHC, HRSA, and HIPAA standards. Support compliance with
immunization tracking, school physicals, and preventive care
metrics. Serve as a trusted healthcare partner for families who may
face barriers to accessing care. Required Skills and Qualifications
The ideal candidate should possess the following skills:
Proficiency in EMR/EHR systems (Epic preferred) Experience in
outpatient psychiatry, crisis intervention, and integrated care
models Knowledge of CPT and ICD-10 coding systems Familiarity with
pain management, substance use treatment, and trauma-informed care
Strong understanding of HIPAA, OSHA, and regulatory compliance
Ability to supervise and collaborate with interdisciplinary teams
Experience in utilization review, discharge planning, and case
management Excellent communication, documentation, and
organizational skills Qualifications Master's or Doctoral degree in
Nursing Board Certification as a PMHNP Active Indiana APRN license
with prescriptive authority (or ability to obtain) Experience in
FQHC or community mental health settings preferred Commitment to
