the Stillwater Hospice Service Area . You will work in a
Hospital-based Palliative APRN role working Monday through Friday,

County). POSITION TITLE: ADVANCED PRACTICE NURSE (APRN) FLSA
STATUS: Exempt REPORTS TO: Chief Operating Officer POSITION
OBJECTIVE: Works in collaboration with the agency medical director
and other team members to deliver hospice and palliative care
services to patients and families under the direction of the COO.
Provides consultation in palliative care, symptom management, and
supportive care to meet patients' needs. Facilitates clarification
of patient and family goals of care. Capable of performing
face-to-face visits for Hospice Patients as outlined in the
Medicare Hospice guidelines for recertification. QUALIFICATIONS:
Graduate of an accredited college with a Master of Science or
Doctorate degree in Nursing Practice. Current license to practice
as an Advanced Practice Nurse (APRN) in the state of Indiana with
prescriptive authority, which includes Indiana controlled
substances registration and a federal Drug Enforcement
Administration registration. Minimum of 5 years nursing experience;
clinical nursing care in hospice, palliative care, oncology,
geriatrics, and/or intensive care preferred. Motor vehicle
operator’s license; access to an automobile with current automobile
insurance coverage. Ability to safely perform the essential
functions of the job without posing a direct threat to the health
and/or safety of themselves or to others in the workplace.
ESSENTIAL FUNCTIONS: Performs bedside palliative care, including
patient evaluation for hospice eligibility. Assists with diagnosis,
treatment, and management of acute and chronic health conditions.
Prescribes medications, including controlled substances, to the
extent delegated and allowed under licensure as an APRN in the
state of Indiana and federal DEA registration. Orders, performs,
and interprets laboratory and radiology tests within the scope of
professional practice. Orders treatments, supplies, and medical
equipment as indicated within the scope of professional practice.
Performs other therapeutic or corrective measures as indicated
within the scope of professional practice. Works under a
collaborative agreement with the Hospice Medical Director. Prepare
and promptly submit all necessary documentation verifying services
provided, including clinical notes, scheduling, patient
evaluations, and related records, in line with Agency policies and
procedures. This documentation will be entered into and maintained
in the patient's medical record. Manage all prescription and prior
authorization requirements for their assigned palliative care
patients. Complies with applicable laws and regulations with
respect to the role of APRN. Educates staff, volunteers, and the
community on all aspects of hospice and palliative care. Adheres to
and implements policies in all work-related situations. Meets
productivity standards as outlined and reviewed annually. Attends
and participates in palliative care team meetings. Works in a
cooperative relationship with other Agency personnel. Seeks
appropriate clinical supervision. Assumes responsibility for
continued personal growth and development. Seeks educational
opportunities to improve clinical practice and participate in
in-service programs. Adheres to Stillwater Hospice privacy
practices as described in the Notice of Privacy Practices. Any
other duties assigned by the supervisor. ESSENTIAL PHYSICAL/MENTAL
FUNCTIONS: Must have visual and hearing ability to comprehend
written and verbal communication- Constant. Must be able to engage

of three (3) flights of stairs. Balancing: Needed for normal

Frequent. Stooping/kneeling/crouching/bending: To perform patient
care - Frequent. Handling objects, i.e., soap, shampoo, toothbrush,
comb, dressing - Occasional. Fingering: Pen, patient documents -

Frequent. Talking: To communicate with patients and family, and to
communicate with supervisors and coworkers - Frequent. Hearing: To
communicate with patients, coworkers, patients’ family, and
supervisor -Frequent to Constant. Seeing: Constant. The ability to
discriminate colors. Must have the ability to remain calm in
occasional emergency situations. Recognizes emergencies and has
knowledge of emergency procedures, according to Agency policy.
ESSENTIAL HAZARDOUS PHYSICAL EXPOSURES: Potential threat of
physical violence to self. (Security personnel available upon
request.) Potential exposure to environmental pollutants. Potential
exposure to infectious diseases, needle sticks, blood and body
fluids, and hazardous waste. Potential exposure to variable
temperatures in the working environment. CHEMICAL OR HAZARDOUS
PHYSICAL EXPOSURES: Potential exposure to substances within the
office environment, including copy machine toner, VDT emissions,
and cleaning. Potential exposure to chemical/cleaning substances in
the patient’s home environment. ESSENTIAL PERSONAL PROTECTIVE
EQUIPMENT REQUIRED: Standard precautions will be consistently used
for all patients. Protective clothing and equipment are provided by
the employer at no cost to the employee. Seat belts must be worn
when traveling during working hours, per state law. OTHER SPECIAL
CONSIDERATIONS: Must be willing to accept assignments without
regard to location, socioeconomic environment, disease, or ethnic
origin. Must exhibit dependability in areas such as attendance,
tardiness, and timely performance of duties. The published
compensation range for this position reflects a good-faith estimate
of starting base pay for full-time (40 hours per week) employment
at the time of posting. The base pay may be higher or lower based
