JAG Healthcare Union City is now hiring for a Registered Nurse (RN) MDS Coordinator! The Registered Nurse (RN) MDS Coordinator will be responsible for conducting and coordinating the completion of the Minimum Data Set (MDS) assessments in accordance with federal, state, and facility guidelines. JAG Healthcare Union City is a 43 bed skilled nursing facility specializing in the care of residents with cognitive deficit disorders with a special attention to those with Geri-Psych and Memory Care needs. Key Responsibilities: Conduct MDS Assessments for all residents in a timely manner, including admission, quarterly, annual, and significant changes in assessments. Collaborate with interdisciplinary team members (nursing, therapy, social services, dietary, etc.) to gather and review information needed for the MDS process. Develop and maintain care plans based on MDS Findings, ensuring they are person-centered and aligned with regulatory requirements. Ensure proper coding of all MDS data and submission to the appropriate agencies (CMS, state, etc.). Review and analyze data for care planning and quality improvement purposes. Monitor and track residents' clinical status and provide guidance to the nursing team regarding any changes in residents' needs. Educate staff on MDS processes, care planning, and compliance standards. Maintain up-to-date knowledge of federal, state, and facility regulations regarding MDS assessments and documentation.
